For those participating in XLR8 Therapy’s mental health counseling or Sports Sponsorship program, we recommend using the weekly and monthly check in form to help track your progress. Not only could this help you succeed in your goals, but it also enters you into the monthly drawing to win one of many great prizes!


 Monthly Prizes:

Each month we will draw from those who have entered into the drawing.  Winners will be notified on the 15th of each month. Entrees will stay in the drawing until drawn or until they are no longer participating in an XLR8 Therapy program.

MONTHLY PRIZES INCLUDE YOUR CHOICE OF: 

  •  XLR8 t-shirt of your choice 
  • 30 minute massage at CSC in Lehi
  • 1 fitness evaluation or re-evaluation
  • 1 box of built bars (your choice of flavor) 
  • 1 month subscription to eMeals
  • 2 boxes of Strikeforce Energy (your choice of flavor) 
  • 2 tickets to your choice of a trampoline park, fun center, swimming pool, bowling alley, axe throwing or golf driving range, etc. (some limitations apply)


WEEKLY CHECK IN FORM:

There are multiple ways to check in every week!  Here are your options:

  • You can purchase the XLR8 therapy journal on Amazon by CLICKING HERE
  • Print off a copy of the journal page and text or email a picture of the completed form (A copy of this is also available in the office):

  • You can also text your therapist your answers to the following questions:
    • Progression questions (1:0%; 6:100%)
      • Compliance to workout plan 1-6
      • Compliance to nutrition plan 1-6
      • Progress on therapy goals 1-6
      • Motivation level 1-6
      • Emotional control 1-6
    • Balance questions (1:not enough; 6:too much) 
      • Stress level 1-6
      • Challenged yourself 1-6
      • Sleep/rest level 1-6
    •  Short answer:
      • Biggest struggles
      • Best achievement
Example answer: 5,6,4,2,3,6,5,1. Candy, 2 less panic attacks this week)

OTHER WAYS TO ENTER INTO THE MONTHLY DRAWING (up to 16 entrees per month!)

  • 1 Entrée per weekly check in submission (limit once per week). Can submit check in form questions via text email response or picture of the filled out form, or filling out the online version.

  • 4 entrees for referring someone who signs up to any of the XLR8 programs
